Jas. D. Davis, a Priv of Captain ______ Company, F of the fourteenth Regiment of Ills Vol was enlisted by ______ of the ______ Regiment of ______ at Jacksonville, Ill on the 23rd day of March, 1865, to serve one year; he was born in ______ in the state of Illinois, is seventeen years of age, five feet two inches high, dark complexion, hazel eyes, brown hair, and by occupation when enlisted, a farmer. During the last two months said soldier has been unfit for duty 60 days. Admitted June 1/65 from Villets Point N.Y. He has no discriptive list in this Gen. Hosp. The above is soldier's own statement.

I certify that I have carefully examined the said J.D.Davis of Captain ________ Company, and find him incapable of performing the duties of a soldier because of (unreadable) Surgeon U.S.A. in charge.
